Healthcare Provider - New (online key and skills testing) Ideal course for healthcare
providers seeking an alternative to classroom training for first time certification. This web based e-learning program used
eSimulation technology to present interactive, realistic patient scenarios. After (part 1) the online portion participants
must meet with an instructor for a practice session (part2) followed by a skills session (part 3). Parts 2 & 3 can be
scheduled for the same day. This online course is not compatible with Mac computers. *Please note that after payment is made you will receive an e-mail

Healthcare Provider - Renewal On-Line Course (Online Key and Skills testing) The Basic Life Support for Healthcare Provider Online Part 1 Course offers a flexible
training option for busy healthcare professionals who would like to either renew their healthcare provider card or for the
first-time user who would like to initially certify for CPR. The complete course is delivered in three parts. Part 1 delivers
the cognitive learning through Web-based, self-paced modules. Parts 2 and 3 require students to meet with an American Heart
Association Instructor to complete a hands-on skills practice session and a skills test. Upon successful completion of all
three parts students receive their healthcare provider course completion card. Information will be e-mailed to you on how to complete the on-line portion after payment is received. When on-line
course is completed, printed out your certificate contact the office to set up the practice/skills portion of the course.You
must show photo ID at skills session. Without a current
card no remediation will be given.*Please note that after payment is made you will receive

Replacement card If you have
lost or missplaced your completion card CPST will gladly reissue you a card for a $6.00 replacement card fee. We
can only replace cards for courses that were taken within our training center. If you are unsure please call 570-954-2729 for us to verify your course. We cannot replace cards
that have expired. In the space below please provide student name, date & location of class, and course you took
(i.e. First Aid, ACLS, PALS, CPR). |

Policies of Center for Public Safety Training
Payment:
All courses must be paid before day of class unless prior arrangements are made.
Refunds: No
refunds will be granted unless course is cancelled by CPST
Class Cancellation: CPST will give
48 hr. notice for cancelled classes, except for inclement weather.
Transfers: You may transfer
once to another class with 24 hr notice.
Replacement Cards: Replacement cards are available
thru CPST for a Fee.
Attendance: You are expected to arrive on time for each course. The instructor
has the right to ask you to reschedule if you are late.
Special Needs: Please notify us in advance if
you have any special needs.
